PROOF OF BIRTH
Enrolling Child’s Birth Certificate, Crib Card, Letter from the hospital
FAMILY INCOME
Household Family Income: (any and all income that any family member receives) this includes: SNAPS, 3 current check stubs (or tax return) Child Support, SSI, Social Security, Retirement, Unemployment, TANF, Disability and self-employment
Documents needed
Social Security Cards for all household members
Form 121 – Shot Record
Medicaid Eligibility Letter
Proof of Residence

All documents can be brought to the center, emailed to a staff member or faxed to 601-947-8223.
A picture of the documents are acceptable as long as they are clear and can be read, please only one item in the the picture at a time, each social security card needs to be taken separately.
